Support workers visit customers in their own homes, usually once a week, with telephone support available when needed. The service operates Monday to Friday, 9am to 5pm.
Each customer works with their support worker to create an individual support plan based on their needs and goals. People can stay with the service for up to three years, or move on sooner if they are ready for independent living.
Support workers help with managing finances, building daily living skills, and developing confidence to live independently. The team also works closely with other services such as offender managers, substance misuse workers, and housing services to provide joined‑up, wrap‑around support.
Who the service is for
This service supports adults who have experienced, or are currently experiencing, rough sleeping and may have found it difficult to keep accommodation in the past. Many people using the service have complex needs, which can include substance misuse, low to medium mental health needs, or an offending history.

- Reduce harm and manage risks
- Access employment, training, or education
- Build confidence and self‑esteem
- Manage physical and mental health
- Learn how to manage a tenancy
- Access local community facilities
Aims and outcomes
The service aims to help customers achieve:
- Greater confidence to manage and keep a home
- Move‑on to permanent accommodation with more independence
- Registration with a GP and improved access to healthcare
- Support with substance misuse or mental and physical health needs
- Improved self‑esteem and wellbeing
Referrals
Referrals are made only through the Rochdale Homelessness Team. Customers must have experienced rough sleeping and had difficulties sustaining accommodation in the past.
