How the service works

The Swale Dispersed Service provides floating support to people living independently in Riverside‑owned properties in the Swale area. Customers live in their own home and receive tailored support through regular visits from a dedicated Support Worker.

The service supports individuals who have experienced homelessness and may have a history of drug or alcohol misuse, physical ill health, mental ill health, or a combination of these. This is designed as a “forever home” opportunity, offering stability and long‑term housing. While housing support funding is provided for up to three years, customers hold an Assured Shorthold Tenancy, giving them security in their home.

Support is focused on helping customers build confidence, improve independence, and achieve long‑term positive outcomes. This includes support with managing money and budgeting, supported shopping, accessing healthcare, and improving overall health and wellbeing. Customers are also supported to engage with statutory services such as the DWP, banks, and adult social care.

Support Workers meet customers in their homes and in the community, empowering them to access services and activities independently over time. Customers are encouraged to build local connections and take part in community activities, helping them feel included and supported where they live.

Formal support planning and review meetings are held regularly. During these meetings, personal goals and aspirations are discussed, and clear steps are agreed to help customers achieve them. The service can also support customers with booking and attending appointments when needed.

Referrals to the service are currently made through the local RSI (Rough Sleeping Initiative) team.
